Scroll down to book a consultation using the form below.

Click the "Continue to Registration" button to complete the new client forms on TherapyAppointment.com.

On the Therapy Appointment site, click the "Register as a New Client" button.

Complete the online forms.

Check your email for a link to create your client portal account username and password on TherapyAppt.com.

Once logged in, click My Docs and Forms from the User Menu.

Complete the remaining disclosures and forms.

You will be contacted within 24 - 48 hours.
